Course Outline
Phase 1 – Identify
Opportunities for performance improvement are identified through a needs assessment completed in the form of a chart review. You will be asked to submit a minimum of 15 patient profiles. Your answers to questions on the profile will be compared with evidence-based standards of care. A gap report is generated showing how your practice compared and how you rank with other similar providers. This report is used to generate a series of learning activities and a performance improvement plan in the Implement phase.
Phase 2 – Implement
Based on your performance improvement plan, you will complete activities tailored to meet your needs and learning preferences for 5 care gaps and you will be asked to implement the lessons learned into your practice. A minimum of 2 activities per care gap and review of 5 tools are required. An extensive reference list and access to proven tools are provided to guide you through the process.
Phase 3 – Integrate
In this phase, you will assess your progress and determine the degree to which you have integrated your learning experience through a second series of chart reviews. An initial set of 10 patient profiles are submitted and a new gap report generated. Following a four-week minimum waiting period, an additional 10 patient profiles are submitted and a final gap report generated.
